SMS Messaging Terms & Conditions

Business Name: Charleston Aesthetic Nurse Practitioners, operating as Charleston Hormone Services (CHS Hormone)

1. SMS Messaging Program

Charleston Hormone Services offers optional SMS (text message) communications to patients who have provided their consent. Our messaging program is intended to facilitate appointment scheduling and administrative communication related to patient care.

2. Types of Messages

By opting in, patients may receive text messages regarding:

Appointment reminders and confirmations

Appointment scheduling and rescheduling

Lab work reminders and completion confirmations

Other administrative communications related to upcoming appointments

Our SMS messaging program does not include promotional or marketing messages.

3. Consent to Receive Messages

Patients may opt in to SMS communications by selecting the SMS consent option within their patient registration and consent paperwork through our electronic medical record (EMR) system, Cerbo.

Participation is voluntary. Consent to receive text messages is not required as a condition of receiving medical care or services.

4. Message Frequency

Message frequency varies depending on appointment activity and individual communication needs.

5. Message and Data Rates

Message and data rates may apply based on your mobile carrier and wireless service plan. Charleston Hormone Services is not responsible for charges imposed by your mobile carrier.

6. Opt-Out Instructions

Patients may opt out of receiving SMS communications at any time by replying STOP to any text message from Charleston Hormone Services.

After opting out, patients will no longer receive messages through this SMS program unless they choose to opt in again.

7. Help and Support

For assistance with SMS communications, reply HELP to any message or contact our office directly.

8. Privacy and Confidentiality

Charleston Hormone Services respects patient privacy and maintains appropriate safeguards for patient information.

Mobile phone numbers and SMS opt-in consent information will not be sold or shared with third parties or affiliates for their marketing or promotional purposes. Information may be shared with service providers as necessary to deliver SMS communications and operate our messaging services, subject to applicable privacy and security requirements.

Standard SMS messaging may not be secure. Patients should avoid sending sensitive personal or medical information through text messaging.

9. Service Availability and Limitations

SMS message delivery is subject to mobile carrier availability, network conditions, and other technical factors. Charleston Hormone Services cannot guarantee timely delivery of all text messages.
